Referral Information and Accepted Insurances

In the state of Ohio you have the choice to select your own physical therapist who can initiate evaluation and treatment without a referral from a physician. The physical therapist will coordinate your care with your primary care doctor or physician of your choice.

We accept most major insurances, Medicare, and Workers Compensation. Please call so that we can verify your individual plan. Although we will work to check your benefits, it is also important for you to contact your carrier to review your benefits for accuracy.

Forms

Below you can find all the forms you need for your first visit with us. All patients, regardless of their insurance, will have to fill out the "New Patient Packet". In addition to the packet, all patients will fill out the corresponding pain rating scale for the place of injury. Patients with Medicare plans or Medicare Supplement plans will have to complete the "Medicare Questionnaire".
